A staged path suggesting the steps of planning treatment abroad

What cancer surgery involves

Cancer surgery is an operation to remove a tumour, and usually a margin of normal tissue around it. It is one of several treatments used against cancer and is often combined with chemotherapy or radiotherapy rather than used alone. The order in which they are given — surgery first, or drug treatment first to shrink the tumour — is decided by the treating team after staging, and it changes the operation that is planned.

The operations differ far more than the single phrase “cancer surgery” suggests. Breast surgery may be a wide local excision, which keeps the breast, or a mastectomy, which removes it, and either can be combined with removal of lymph nodes from the armpit — a sentinel node biopsy, or a fuller axillary dissection. Bowel surgery removes a segment of colon or rectum and usually rejoins the two ends, and some bowel operations involve a stoma. Head and neck, thyroid, gynaecological and urological cancers each have their own named operations, with their own recovery.

In theatre the tumour is removed and sent to a pathology laboratory. The pathologist reports the type of cancer, how it looks under the microscope, and whether the edges of the specimen are clear. That report is what the next decision rests on — whether further surgery, chemotherapy or radiotherapy is recommended.

Reconstruction can be done at the same time or later, and it is a separate decision with a separate surgeon.

When cancer surgery is considered

A surgical opinion is usually sought at one of a few points in a patient’s pathway. Each of these situations is assessed individually, and none of them is a rule that a web page can apply to your case.

  • A scan or a biopsy has shown a solid tumour that appears removable
  • Staging imaging is complete and the extent of the disease is known
  • Drug treatment or radiotherapy has been given first, and the team is reviewing what is left
  • A blockage, a bleed or a similar problem needs to be relieved

The decision belongs to a multidisciplinary team — surgeons, medical and radiation oncologists, radiologists and pathologists reviewing the same scans and reports together. What can be done from Dhaka, in advance, is to assemble the file: the histopathology report with slides or blocks if they can be released, imaging on disc, the staging report, recent blood results and a medication list. A hospital abroad will rarely price anything without them.

Cost by destination

No Bangladeshi hospital publishes a cancer surgery price, and no Indian hospital we checked publishes resection prices either. Where a hospital does print figures they are for named operations rather than for “cancer surgery” as a whole — so the Thailand and Singapore rows are split by operation and must not be read as one general price.

The Bangkok page prints no inclusion or exclusion list, so read those figures as covering the operation and nothing else. The Singapore figures are medians of actual 2023 private-hospital bills, not packages, and none of these numbers includes flights, visa or treatment after surgery. Where a cell reads “quoted per case”, we will ask the hospital for a written figure against your reports — the same request, in writing, is what makes two quotes comparable.

Recovery timeline

What follows is the usual sequence for uncomplicated breast or thyroid surgery; bowel surgery takes longer. Every step depends on the operation and on how recovery goes, and the discharge decision is the treating team’s.

  • Days 1–3 — ward care, pain control and getting mobile; drains are common after breast surgery
  • Days 3–7 — wound and drain checks; many breast and thyroid patients are discharged inside this window, bowel surgery usually is not
  • Days 7–14 — a fit-to-fly assessment; a letter is usually needed, and flying is normally delayed after chest or abdominal surgery
  • Weeks 2–4 — pathology review and the decision on further treatment, which can often be taken with your oncologist in Dhaka if the records are shared
  • Weeks 4–12 — a graded return to activity, with physiotherapy after breast or abdominal surgery

Two things decide how long you actually stay: when the pathology report is expected, and when the surgeon will sign you off to fly. Ask for both dates in writing before booking — changing a return flight costs more than most families budget for.

Risks and what can go wrong

All cancer surgery carries the risks of any operation under general anaesthetic, plus risks specific to the site. What follows are categories, not probabilities — the consent discussion with your own surgeon is where they are weighed for your case.

  • Bleeding, and the possibility of a transfusion or a return to theatre
  • Infection in the wound, or deeper in the operation site
  • Wound breakdown and slow healing, which is more likely after radiotherapy
  • Fluid collecting under a breast or armpit wound, which may need draining
  • Arm swelling (lymphoedema) after lymph nodes are removed from the armpit
  • Blood clots in the leg or lung around the time of surgery and the flight home
  • A stoma, temporary or permanent, after some bowel operations
  • Hernia or adhesions later, after abdominal surgery
  • Scarring and a change in appearance, with reconstruction as a separate decision
  • Pathology showing more extensive disease than the scans suggested, so that further treatment is recommended

One risk is specific to operating abroad: a complication that appears after you are back in Dhaka. Ask before you commit what the hospital’s policy is on readmission, who is responsible for a wound problem at home, and whether the quote covers a return to theatre — in writing.

Frequently asked questions

Can I get one fixed price for cancer surgery before travelling?

Usually not. A meaningful quote names the operation — “mastectomy with axillary dissection”, not “cancer surgery” — and the operation is not fixed until staging and the pathology review are complete. Ask for an itemised written quote naming the procedure, the number of nights, and what happens financially if more surgery is needed.

How long will I need to stay in the destination country?

Long enough for discharge, for the pathology report to come back, and for the surgeon to sign you off to fly. For breast or thyroid surgery that is often around two weeks; for bowel surgery it is longer. Ask for both dates before you book.

Should the biopsy be done in Dhaka or abroad?

Most patients arrive with a biopsy already done in Dhaka. Some centres repeat tests, or want the original slides and blocks couriered, and that takes days. Ask what the receiving hospital needs before you book a flight.

Can my oncologist in Dhaka stay involved?

Yes, and it is worth arranging. Ask for a discharge summary and the full pathology report addressed to your doctor at home, and keep your own copies; follow-up is usually shared.

Does the quoted price include treatment after surgery?

Almost never. Chemotherapy, radiotherapy and follow-up are billed separately, and much of that can be given in Dhaka. Ask which parts of the plan the hospital expects to deliver and which your local team takes over.

What if the visa has to be arranged quickly?

Medical visas are usually expedited against a hospital appointment letter. Our medical visa guide lists the document set we ask patients to prepare.

Next step

Send us the pathology report and the imaging you already have, and tell us what the treating doctor in Dhaka has said so far. We will put the file to two or three hospitals and come back with written, itemised figures for the operation that is actually being proposed.
